I acknowledge that I volunteered to participate in the advertising and to have my name, likeness, and private facts featured in the advertising materials. I also acknowledge that my participation in the advertising and the inclusion of my name, likeness, and private facts in the advertising materials are not a requirement or condition of my employment.

I agree that Flyland Recovery Network will be the sole and exclusive owner of all rights in and to the advertising materials. I grant Flyland Recovery Network the right to use, incorporate, distribute, re-use, publish, re-publish, alter and/or edit the advertising materials in perpetuity, for unrestricted use in any form of media and for any means of promotion, merchandising, publicity and advertising.

I agree that the inclusion of my name and likeness in the advertising constitutes a good and valuable consideration. I agree not to seek any consideration or compensation for the use of my name, likeness, or private facts in the advertising materials. I hereby waive any right that I may have to monetary compensation (royalty-free) or to inspect or approve the finished product or the advertising or other marketing uses made of the product.

I agree to indemnify and hold Flyland Recovery Network harmless with respect to the use of my name, likeness, and private facts in the advertising materials. I expressly release Flyland Recovery Network from any and all legal claims that I have or may have arising out of the advertising materials.

I waive any right to inspect or approve the finished version(s), including written copy that may be created in connection with the content. I know I can notify Flyland Recovery Network in writing at any time should I wish to be removed from the advertising. I understand it can take up to six months to replace advertising that includes my name and likeness. 

This agreement contains the full and complete understanding between the parties, supersedes all prior agreements pertaining hereto, and cannot be modified except be a writing signed by both parties. This agreement will inure to the benefit of and be binding upon the parties’ respective successors, licensees, and assigns. 

This agreement will be governed by and construed in accordance with the laws of the State of Florida. I hereby consent to the exclusive jurisdiction of the courts closest to the city of Palm Beach, Florida with respect to any matter arising out of or relating to this agreement.

I certify that I have read the foregoing and fully understand the meaning and effect thereof, and I intend to be legally bound.
